HealthReach Community Clinic provides primary medical care,  a pharmacy, an on-site lab, preventive care, wrap-around services, and referrals for diagnostic imaging and specialty care.

Primary Care

HealthReach Community Clinic offers primary care medical services including non-emergency acute medical care, care of chronic diseases, and limited diagnostic lab and imaging.

Pharmacy

Our pharmacy fills prescriptions written by our doctors and other medical providers for patients seen at our clinic. Prescription medications filled in our pharmacy are free for our patients. There is a charge for Diabetic Test Strips and supplies. We do not carry narcotics or any other controlled substances in our pharmacy.

If the patient requires a drug that we do not carry or needs a continual supply of a drug for an extended period of time, we provide assistance with enrollment in Prescription Assistance Programs (PAPs). Many pharmaceutical companies have PAPs, which allow people to get their prescription drugs at no cost. However, the process can be very paperwork-intensive. We assist our patients through the process, helping them with both the application and ongoing processes.

If a patient needs to consult with our pharmacist, she is available Tuesday, Wednesday, and Thursday. Patients who DO NOT need a consultation with our pharmacist may drop off or pick up prescriptions any time the clinic is open.

Labs

Diagnostic services are supplemented by the use of an in-house lab where blood and urine specimens are collected. Specimens are analyzed using a local lab and results are returned to your HealthReach provider for interpretation and diagnosis.

Preventive Care

Prevention and early detection of disease are fundamental to our work. HealthReach offers free health screenings and well visits for all patients and regularly monitors patient health through blood pressure, blood glucose, BMI, hemoccult, mental health questionnaires, and more. Included in our women’s health services are annual PAP tests, breast exams, labs and screening for personal safety. We also provide free flu vaccines, nutrition counseling, weight management, and smoking cessation counseling, replete with pharmacotherapies. Thanks to our clinical partners, we offer free mammograms, pulmonary function tests, retinal imaging, and even free lung cancer screening.

Specialty Care

HealthReach sees many people who need care that we cannot provide directly. Many of our patients need the help of a specialist or a surgeon to address their issues. We partner with local specialists to help our patients obtain these services but cannot guarantee services will be provided at no cost.

Wrap-Around Services

In addition to medical services, HealthReach’s social worker coordinates important wrap-around services for patients.  Our social worker is available to meet with individuals and families to address a number of issues ranging from emotional health needs to referrals for additional community resources. Patients should ask to schedule an appointment to meet with her.
